By doing this, it means more visits and better care at the bedside to provide comfort and support. Our entire team, including physicians, nurses, and more, work hard to keep our patients at home and to avoid ER visits and rehospitalizations. Our visits remain frequent and focused with an emphasis on quality time and excellent communication with patients and families.

Community Collaboration

We are proud of the strong relationships we have formed with other healthcare facilities and the medical community of the Dallas area. Our hospice patients in Assisted Living or Long- Term Care communities receive excellent end-of-life hospice care through our partnership, coordination, and collaboration with facility staff. To meet the needs of our patients and families, we take the necessary time to communicate effectively with our facility partners.

At Home When It Matters

If there is a decline with our hospice patients and more support is required, we increase our patient home visits and have the ability to provide 24/7 continuous care support if it is deemed medically necessary. Because of the low caseloads our staff carry, they are able to increase their home presence to support our patients and families when they need it most. When you need us, we are there.

After Hours Support Team

Our dedicated after-hours on-call team is there when situations arise for our patients and families that require 24/7 support. We also have a 24-hour call line that is supported by an experienced Nurse Manager and field staff to provide patient and family support at any time, day or night. When you need us, we are there. Our goal is to provide comfort and peace, no matter what time it is.
